  • Press 4 if you need to talk to someone about pre-authorization.
  • Press 5 for all other questions/purposes.

What to Expect

Every patient's process may vary, but this is what you can generally expect.

  1. You are generally referred to us by your doctor. Please call us to schedule an appointment.
  2. You choose one of our offices closes to you.
  3. You are assigned a mid-level physician.
  4. Be sure to bring your insurance, medical history, procedure details, allergies, and medication list with you.
  5. You are seen on your appointment day.
  6. Testing is done, if needed.
  7. If further procedures are needed, you are given educational materials and your doctor will know when it’s appropriate to schedule it.

New patients are always welcome

Please download, fill out and bring with you to your first appointment.

Same day or next day appointment requests are allowed based on availability.

Our physicians will not see you as a new patient without medical records indicating why you are being seen.   We don’t want to waste your time or your money by having you here and not being able to accomplish what you need.   Without information about why you have been referred,  recent  lab or diagnostic testing,  decisions cannot be made. Please work with us on this very important item and make sure all of your information is here before you come in for an appointment.

  • If you are a Tricare patient,  you will always need a referral to be seen. Please be sure there is a referral in place before coming to your appointment by calling your Primary physician.
  • If you are a Veteran,  you will also need to have a referral to be seen. Please be sure that there is a referral in place before coming to your appointment.
  • If your insurance requires a referral to a specialist,  and there is not one in place,  you may be responsible for the bill.

Cancellations/Late Doctors

Please read our late policy.

  • If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another person.
